St Augustine Youth services is a well-known and respected organization that has been providing residential therapeutic care to boys for over 30 years. We're looking for a PRN/part time RN/LPN to help support our team. The ideal candidate will have at least 2 years of experience working with youth and will be familiar with care for patients with a mental health diagnosis.
Registered Florida license in the field of nursing (RN or LPN)
A valid Florida driver’s license
Personal and professional reference checks are required
A history free of offenses as listed on the Affidavit of Good Moral Character
Must be able to pass an extensive background screening
Must be able to pass a drug screening
Must have a passion for helping others
Present oneself in a positive, professional manner to the children and other agency personnel
Must possess excellent oral and written communication skills
Be a passionate role model and remain a positive mentor for children
Be a productive Team Member. Desire to work cooperatively as a member of a team for the benefit of the child and the organization
Prior positive interactions and experience with children is preferred
Perform work on an as needed basis
Provide the five functions of care management as needed: planning, assessment, linking and advocacy, monitoring, and discharge planning
Participate with admission/intake and discharge planning as needed
Provide health and medical referrals, health education, and basic health screenings
Participate in medication reviews, coordinating medication needs, and maintaining/updating medication documentation
Refer and coordinate medical services that may include coordination initial appointments and follow ups
Maintain electronic health records accurately and in a timely manner
Maintain knowledge of involuntary psychiatric evaluation procedures
Maintain a list of community providers for referral services
Medication training and review of medication charts
Complete health screenings
Work with the psychiatrist during medication review as needed
Dispense medication as prescribed by the doctor
Oversee medical appointments and document on the EMR system
Perform other duties as required
Remain available for weekends on call as needed
Assist with fundraising efforts
