Lasell Village is a life plan (CCRC) community ofabout 200 residents in Newton, Massachusetts. Additional health care options for residents include a skilled nursing facility (Lasell House) and a supported living facility (Lasell Studios). The Wellness Center Social Worker is responsible for identifying the social, psychological, and emotional needs of residents in independent living, and Lasell Studios, and for making recommendations and arrangements for services to meet those needs. The position includes time spent with families and residents in the Wellness Center, as well as documentation, meeting and related conditions. Quality customer service is a top priority for everyone at the Village.
RESPONSIBILITIES:
· Conduct comprehensive assessments of residents’ physical, emotional, social, and environmental needs to identify areas where support may be required
· Regular assessments of residents need to be conducted 2x/year, or more frequently as needed to ensure appropriate services are provided.
· Participates in resident admission and discharge and provides necessary documentation, as needed.
· Coordinates transfer of resident from one level of care to another, actively participates in transitional care.
· Documents and charts care plans, assessments, results, meetings, and changes.
· Facilitates group sessions based on the needs of the community.
· Participates in Village Club along with other specialty groups such as hearing/vision impairment groups.
· Works cooperatively with Social Worker at Lasell House to provide a continuum of care.
· Develops and maintains community resources and contacts
· Assists residents with Medicaid/Medicare concerns.
· Keeps educational credentials and professional certificates current
· Develops goals and objectives annually
· Develops and maintains outside professional contacts
· Maintains confidentiality and protects sensitive Protected Health Information (PHI) at all times
· Promote the Mission, Vision, and Values of Lasell Village in all areas of responsibility
SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
· Masters in Social Work is preferred
· Minimum requirement is a four-year college or university education with a B.A./B.S. degree in group work, psychology, counseling, social work, or related field.
JOB SKILLS:
· Five years’ experience in a social services role
· Must possess excellent communication, leadership, and organizational skills, and have a high degree of flexibility and adaptability
· Must be able to assist residents and staff in case of a disaster
· Must have excellent interpersonal and communication skill
· Thrive in a team environment and work well with others
ENVIRONMENTAL FACTORS & PHYSICAL DEMANDS:
· May need to ascend/descend stairs, balance, stoop, kneel, crouch, crawl and twist
· May need to work in protective clothing, when company policy and common sense so dictates
· Frequent sitting, but moves intermittently during working hours and is subject to frequent interruptions
· Works beyond normal working hours and on weekends and holidays when necessary
· Is subject to call-back during emergency conditions (e.g. severe weather, post-disaster, etc.)

Pay: $73,000.00 per year
