Responsibilities:
· Advises prospective adoptive parents as to options within the Caring Adoptions adoption program;
· Conducts information sessions to educate clients that are interested in working with Caring Adoptions;
· Keeps current contact with clients in post placement in the program monthly;
· Facilitates parent education classes for prospective adoptive parents in the adoption program
· Secures all documentation from the adoptive family as required by Texas Minimum Standards for post placement reports, updates, etc.;
· Maintains individual files for each client in the post placement process;
· Maintains current files for each individual family throughout the post placement process;
· Coordinates with birthparent case manager for placement time;
· Assists with placement documentation at the time of placement and is a witness as needed;
· Coordinates placement with adoptive family, including paperwork for the placement;
· Tracking, reviewing, and filing monthly progress reports, pictures, and medical reports, etc. submitted by the post placement family
· Maintenance of the managing conservatorship binders
· Ensuring the provision of the finalization packet to the post placement family at the time of their final post placement visit
· Being apart of the Child Placing Management Staff for approvals
· Facilitation of in-person meetings between adoptive families and birthparents, as requested
· Obtaining Birth Certificates, decrees and other documents as needed
· Complete all reports in a timely manner
· Working with other members of the team to complete tasks
· Completing HSEGH (Health, Social, Educational, Genetic History reports) and redacting medical records
Requirements:
- Bachelor's degree in social work, child development and family living, human sciences or a related field is required.
- Experience as a case worker or social worker is preferred.
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent organizational and time management skills
- Self starter
- Knowledge of Texas Minimum Standards for Licensed Child Placing Agencies preferred, but not required
