LVN/RN Clinical Intake Specialist
We are seeking an LVN/RN Clinical Intake Specialist to join our constantly growing team. The Intake Specialist will be responsible for communicating with patients and referral sources in a professional manner, ensuring quality patient care, maintaining accurate communication and care records, and implementing new standard operating procedures in our dynamic, growing company.
About our Company
Adaptive Home Health is a privately owned Home Health Agency licensed by the State of Texas. Adaptive Home Health specializes in high acuity home health services throughout North Texas.
Responsibilities include:
- Review incoming referrals and ensure all documentation is received before acceptance with referral source
- Review patient appropriateness for acceptance based on agency policy
- On-call once every 5 to 6 weeks (remote)
- Communicate with field staff to address patient care concerns
- Answer and respond to incoming calls to the clinical phone line
Required Qualifications:
- 5 years of healthcare experience
- Prior intake or admissions experience
- Strong attention to detail
- Current CPR certificate
- First Aid training
- Valid driver’s license
- RN/LVN license in good standing
Preferred Qualifications:
· Ability to sustain efficient, timely, and accurate work in fast-paced work environment
· Strong experience with Microsoft Office

The Clinical Intake Specialist position requires working within our normal business hours of 8:00 AM - 5:00 PM. However, this position will require after-hours and weekend availability as needed.
Job Type: Full-time
Pay: $50,000.00 - $60,000.00 per year
