Job Summary
Partner in Excellence (Partners) is a health care provider supporting children in Minnesota and Wisconsin areas. Partners services include intensive autism therapy services, occupational therapy, speech therapy and children’s mental health targeted case management. Case managers assist children and families in gaining access to medical, social, educational, vocational, and other necessary services to decrease mental health symptoms. Case managers work closely with families, children, and other community professionals.
Essential Functions
- Review and understand medical documentation, including, but not limited to diagnostic assessments, discharge summaries and other mental health or medical records.
- Collaborate with client/family, professionals, family’s community supports/network in a nonjudgmental and safe way.
- Strong clinical writing skills
- Assist the client/family in obtaining needed services by referral to and coordination with other agencies/providers.
- Assure continuity of care and continually assess the effectiveness of services.
- Provide wrap-around support and care via coordination of all community provides and natural/informal supports.
- Strong interpersonal skills to ensure positive work relationships with many different personalities.
- Maintain professional working relationships with clients, providers, co-workers and supervisors.
- Ability to problem solve and make appropriate decisions.
- Ability to both work with a team and independently.
- Demonstrates dependability and consistency.
- Document client/family contacts accurately and in a timely fashion
- Maintain client files per state and county regulations.
- Ability to use time effectively, work under stress and intervene appropriately in crisis situations.
- Complete required training and participate in ongoing training opportunities as instructed.
- Be fully knowledgeable of Partners in Excellence’s Privacy Policies and Procedures and report any violations to Center Manager and clinical supervisor. Protect Partners’ clients’ privacy at and away from the site.
- Appropriately receives feedback and implements changes in a prompt and positive way.
- Flexible work schedule, including overtime, nights and weekends as required, including attending events/meetings as needed (must be available to work 5 days a week)
- Complete all other projects/duties as assigned.
Required Education and Experience
- A Bachelor’s degree in one of the behavioral sciences or related field and has a minimum of 2000 hours of clinically supervised experience providing services to children with Severe Emotional Disturbance OR is qualified as a mental health practitioner due to a master's or other graduate degree in behavioral sciences or related fields.
- Experience and training in identifying and assessing a wide range of children’s needs
- Knowledgeable about local community resources and how to use those resources for the benefit of children and their families.
- Meet supervision and continuing education requirements.
- Successfully pass initial and subsequent background checks (no history of felonies, substantiated client maltreatment or vulnerable adult incidents, or disqualifying misdemeanors)
- Successfully complete the Minnesota Department of Human Services (DHS) MH-TCM curriculum training

Work Environment*
This job primarily operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must also be able to travel in MN weather conditions to see clients in home/community setting.
