TOUR 2: (9am-5pm)
Purpose of Position:
To provide psychosocial assessments and the completion of the HRA2010e for homeless HIV patients.
Areas of Responsibilities:
All HIV patients that are in need of permanent, safe housing that are within the CHAT Program (Comprehensive Health Assessment Team).
Summary of Duties and Responsibilities:
1. Serves as a member of interdisciplinary team to assess housing needs of persons with HIV related illnesses.
2. Will assess clients off site as necessary.
3. Has ability to work remotely at least two days per week.
2. Formulates psychosocial evaluations of clients.
3. Prepares HRA 2010E Housing applications and utilizes PACT Program.
4. Provides concrete services and referrals as necessary.
5. Documents all interventions and activities in the patient’s electronic record in accordance with Departmental policies.
6. Maintains statistical data regarding interventions using Microsoft Excel, Microsoft Access and HASA Web.
7. Attends necessary meetings in both Social Work and Comprehensive Health Assessment Team.
8. Performs other related functions as determined by Supervisor.
