Blue Hills Therapeutics, Inc. is seeking Per Diem RNs and LPNs to care for individuals in their home. Healthcare is changing and more and more people are requiring care in their homes to manage their various disease processes. Responsibilities: The Home Health Nurse delivers professional nursing services in the patient's home setting:
- Performs a comprehensive health assessment on patient/family, including them in the goal-setting process and discharge planning.
- Provides hands-on care, case management, medication updates, evaluation & reevaluation of the care plan and teaching of the patient, family, caregiver in accordance with physician orders as required by our policies and other regulations.
- Establishes realistic patient-care goals, with measurable outcomes.
- OASIS assessments as appropriate
- Initiates appropriate preventive and rehabilitative nursing procedures, making necessary referrals to community services as needed.
- Care Coordination
- Ensures quality and accuracy of patient records and charts in compliance with Medicare and Mass Health standards.
- Responds promptly to emails and phone calls from care team.
- Report patient progress and condition to patient's physician and Clinical Manager/ Director of Nursing on a continuous basis.
- Coordinate care with interdisciplinary staff members to include Physical, Occupational and Speech Therapists; Home Health Aides, Personal Care Attendants.
- Maintains Agency property, supplies and equipment in a manner that demonstrates ownership and accountability.
- Demonstrates infection control per OSHA guidelines and Agency policies.
- Ensures compliance with the Medicare Conditions of Participation and any other regulatory requirements.
- Participates in staff meetings, conferences, and other training per state requirement.
- Maintains professional appearance, treats others with care and respect, and demonstrates excellent customer service.
- Demonstrates overall skill in caseload management.
- Ensures required productivity standards are maintained.
- Initiates case conference review for difficult or high-risk cases.
- Identifies ancillary services and supplies needed by families and coordinates and/or supervises these services.
Education and Experience Requirements:
- LPNs with Current MA license
- One or more years of current nursing experience in certified home health care.
- Able to Sit, stand, bend, lift, kneel, climb stairs, and move patients safely as required.
- Ability to work in the patient's home environment
- Participate in continuing educational programs to stay current on home health practices, laws and regulations.
- Demonstrate strong verbal, written and interpersonal skills to communicate effectively with staff, nursing, and administrative team.
- Must have auto, driver's license, and current auto insurance.
- Fluent English with skills in a second language is a plus.
- Computer skills to perform documentation of visits.
- Performs other duties as assigned.
