Omni Wound Physicians is looking to hire a case manager MUST have LVN or RN license. Case managers are centralized around assessing, monitoring and evaluating the patient status. This role will include, coordinating abdominal KUB's, G tub replacements, along with lab orders, call pharmacies for patient medication. In this role, you will be setting up appointments, managing provider's schedule, and informing participants of rescheduled or canceled visits in a timely manner.
To ensure success, Case Managers should possess solid knowledge of scheduling practices and demonstrable experience in a similar role. A first-class scheduling coordinator will be someone who takes proactive steps to initiate seamless scheduling processes.
Case Manager Responsibilities:
- Coordinate abdominal KUB's and Gastrostomy tube replacements for urgent needs and routine replacements
- Coordinate lab orders
- Appropriate follow up with pharmacies on medication orders.
- Coordinate with the Scheduling team to manage provider's schedule.
- Maintaining and updating Wound Ally with the highest level of accuracy as needed
- Monitor patient hospitalization lists.
- Maintain and updating schedules, calendars, and agendas.
- Confirming appointments and obtaining patient's consents.
- Sending out reminders of scheduled appointments.
- Answering scheduling queries via email and phone.
- Performing other administrative tasks when required.
- Monitor patient hospitalization lists.
- Ensure Durable Medical Equipment orders are processed
- Performs other duties as assigned.
Case Managers Overview:
- Must have LVN or RN license.
- A minimum of one year experience in a similar role.
- Advanced knowledge of confirming and rescheduling appointments, as well as recordkeeping practices.
- Strong critical thinking skills and the ability to solve scheduling challenges with in light of stringent deadlines.
- Extensive experience with office software, including MS Word, Excel, and Dropbox would be an asset.
- A Bachelor degree in a health care related field would be an asset.
- Excellent organizational, interpersonal, and communication skills.
