POSITION SUMMARY: The Case Manager - Long Term Housing position supports DHS involved families, navigating a range of challenges, to work toward self-sufficiency. The position is deeply connected with families involved with the child welfare system. Through intensive case management and family support, the goal is to provide services to help stabilize families as they move toward self-sufficiency and raise safe and healthy children. The Family Nurturing Center Programs strengthen families through the development of Protective Factors, which reduce the risk of child abuse and neglect and support effective parenting in at-risk populations.
MAJOR RESPONSIBILITIES:
- Supporting families as they navigate DHS Child Welfare System
- Reporting safety threats
- Maintaining compliance with FNS/DHS contract
- Fostering relationships with community resources
QUALIFICATIONS & EDUCATIONAL REQUIREMENTS:
- High School Diploma
- Minimum 2 years’ experience working with high-risk families (within past 5 years)
- Current clearance in DHS and Childcare licensing background check registry
- Current Recognizing and Reporting Child Abuse and Neglect (within 30 days of employment)
- Current First Aid/CPR Card (within 30 days of employment)

PHYSICAL REQUIREMENTS:
- Able to effectively handle crisis situations
- Ability to lift up to 40 pounds
- Sitting, standing, walking, driving
- Normal memory, taking into consideration the amount and type of information
- Vision and hearing within normal range (with or without corrections)
HAZARDS:
- May come into contact with children who are ill and/or contagious
- Entering potentially unsafe, unstable, or unsanitary living conditions
- Driving in potentially hazardous weather or road condition
Specific Responsibilities:
Supporting families as they navigate DHS Child Welfare System
- Managing a caseload, including daily check-ins, with families living in housing supported by the Family Nurturing Center Coordinating with providers and workers involved with families.
- Working with families to support self-sufficiency
- Teach families how to access community resources
- Being available and attentive when families need assistance
- Supporting families in their sobriety, obtaining employment, and acting as a safety net resource for families
- Transporting clients as needed.
Reporting Safety threats
- Assessing the safety of housing
- Providing crisis intervention, including referrals to community partners, accessing services and advocating in court to meet family needs.
Maintaining Compliance with FNC/DHS contract
- Administering family assessments
- Regular reporting to DHS Child Welfare, and maintaining accurate records and documentation.
- Completing semi-annual developmental assessments with families
- Participating in regular reflective supervision with supervisor
- Participating in training, workshops and seminars
Fostering relationships with community resources
- Support in negotiating leases and rental agreements
- Understanding local community resources for families
Job Type: Full-time
Pay: $18.93 - $20.89 per hour
