Home Visitor: Maintains a caseload of 20-25 families to provide home visiting services for children and their families who meet the eligibility requirements for “at risk” as defined by the Help Me Grow program policies. Home visits include the development of an Individualized Family Service Plan, ongoing child and family assessments, delivering parenting/child development curriculum, assisting families in securing needed services, and helping families resolve barriers to achieving self-sufficiency.
Principle Duties and Responsibilities:
- Conducts initial intake and determines eligibility for services in the Healthy Moms & Babes Help Me Grow Program
- Completes all necessary and required documentation in accordance with Ohio Department of Children and Youth and Help Me Grow policy
- Conducts regular home visits to provide developmental and other screenings, develop, monitor and review the Family Goal Plan and deliver a parenting/child development curriculum
- Completes developmental screenings within the required time frame and with the consent of the parent
- Helps families identify and establish a medical home
- Provides assistance in accessing community resources such as food, housing, utilities, etc.
- Advocates for families when necessary and acts as a liaison between the family and other service providers
- Assists in coordinating transition to other programs and services as children near age three or earlier as needed
- Participates in outreach activities to identify new clients and educate the community about the services available, including staffing health fairs
- Maintains records of the child and family in a confidential and up-to-date manner including documentation in the Healthy Moms & Babes Database and OCHIDS Database
- Attends required DCY Help Me Grow training
- Complies with all State of Ohio Help Me Grow Policies and Procedures
- Complies with all Healthy Moms & Babes policies and procedures
- Attends regularly scheduled staff meetings and supervision sessions
Knowledge, Skills, Experience Required:
1. Culturally competent in working with families of diverse and socioeconomic backgrounds
2. A working knowledge of typical and atypical child growth and development
3. Sensitivity and awareness of disability issues
4. A working knowledge of theories and principles concerning family empowerment and working with families
5. Ability to communicate effectively and clearly verbally and in writing
6. Skills in interviewing, negotiating and advocating
7. Ability to locate and effectively utilize services
8. Two years of experience working with children birth to three years and their families preferred
9. Must have reliable transportation, good driving record, valid driver’s license
10. Certified Community Health Worker (CCHW), Associate's Degree, Bachelors Degree in education or social service field.
