**Job Overview:** We are seeking a dedicated and compassionate Case Manager to join our dynamic team. The ideal candidate will have a strong foundation in social work and be passionate about supporting vulnerable populations. The Case Manager will play a vital role in providing individualized support to clients, connecting them with essential resources, and advocating for their needs. **Responsibilities:** - Provide case management services to clients, including assessments, goal setting, and monitoring of progress. - Develop and maintain relationships with clients, conducting regular home visits and phone calls to ensure their well-being. - Identify and address barriers to healthcare, social services, and employment, utilizing motivational interviewing techniques. - Collaborate with multidisciplinary teams, including healthcare providers, social workers, and workforce development specialists, to ensure clients receive comprehensive care. - Facilitate crisis intervention and management, connecting clients with emergency services as needed. - Provide behavioral health support and referrals to clients, ensuring they have access to mental health resources. - Utilize knowledge of child welfare systems to support clients navigating these complex services. - Stay up-to-date with Section 8 housing programs and assist clients with housing applications and maintenance. - Work with clients to develop and implement personalized plans, including education and job training. - Conduct needs assessments and provide referrals to clients for essential services, including childcare and workforce development programs. **Experience:** - Bachelor's degree in social work, psychology, or a related field. - Proven experience in case management, preferably in a healthcare or social services setting. - Strong understanding of behavioral health principles and motivational interviewing techniques. - Knowledge of Medicare and Medicaid programs, as well as Section 8 housing programs. - Experience working with diverse populations, including children and families. - Familiarity with child welfare systems and crisis management techniques. - Ability to work independently and as part of a team. - Excellent communication and interpersonal skills. - Compassionate and empathetic approach to client work.
