Description:
AA Family Hospice is looking for an experienced CHHA with open hours and flexibility. Someone who is willing to drive to multiple areas for patient care. Performs various patient care activities and related non-professional services necessary in caring for the personal needs and comfort of patients. Must be flexible with driving to different areas for patient visits.
Responsibilities:
- Helps patient with bath, care of mouth, skin, hair and/or other personal hygiene activities.
- Helps patient in and out of bed.
- Assists patient with ambulation use if wheelchair and/or walker as directed.
- Helps with prescribed exercises.
- May prepare meals and help patients with eating.
- Reinforces therapeutic diet instructions.
- Performs household services, which facilitates the patients self-care at home and are necessary to prevent or postpone institutionalization.
- Helps patient to bathroom or in use of bedpan/urinal.
- Reports changes in patient’s condition and/or needs to supervise nurse and appropriate therapist.
- Works independently in home with only intermittent RN supervision.
- Schedules own home visits.
- Assists patient in taking medications that are normally self-administered.
- Charts record of patient care on progress notes after each visit.
- Completes other appropriate records.
- Provides for a safe and sanitary environment for the patient adheres to Calstro Hospice Los Angeles. Infection Control and Safety Management policies.
- Participates in and/or conducts team conferences.
- Participates in other specific duties as assigned and defined by Calstro Hospice Inc. policy.
- Performs all activities incorporating and applying the principles of proper body mechanics.
- Participates in orientation, staff education, team training, team meetings and patient care.
- Implements care based on Plan of Care and HOSPICE policies.
Work Experience, Skills and Education:
- Six (6) months of verifiable aide care in hospice setting
- Successful completion of a Home Health Aide training program as recognized by the State of California for certification.
- Current Home Health Aide Certificate issued by the State of California.
- Current CPR certification.
- Current Drivers License
- Current car insurance coverage
- MUST be comfortable working in facilities and following facility protocols.
- Successful completion of self study modules on Infection Control, Fire/Safety, Body Mechanics and Emergency Preparedness with a score of 70% or more.
- Bilingual in English and Spanish preferred
