We are seeking to hire an experienced Social Services to join our team.

Responsibilities:
- Complete initial assessments
- Assist with discharge planning
- Document patient progress and coordinate family meetings
- Ensure regulatory compliance
- Complete MDS sections as assigned, ensure all areas are addressed in the care plan as necessary.

Requirements:
- A bachelor's degree in social work or a closely related field
- State licensure preferred (CSW, ACSW, LSW, LCSW)
- Interpersonal, critical thinking, and organizational skills
