Case Manager
Position Summary: BEDS Plus provides services to remediate homelessness in southwest suburban Cook County. We are a 35 year old organization with roots in La Grange. Our mission is to help people obtain and retain housing. We do this by working with those is need to build and work on plans which remove the barriers to accessing housing. Working closely with our clients, BEDS case managers are often successful in providing the assistance needed to access housing. The case manager will oversee and deliver case management services to to those assigned to them. Currently, we have three case manager openings in various lines of work, including domestic violence, permanent supportive housing and interim housing.
Primary Responsibilities:
Case Management
- Qualify individuals and families for intensive case management services, working cooperatively with other BEDS staff and their managers.
- Consult across program areas (shelter, prevention and housing) to meet the unique needs of homeless individuals and families.
- Create and document individualized client goal plans for BEDS clients.
- Connect BEDS clients with government benefits, medical providers, mental health services and housing options.
- Collaborate with other service providers to connect clients with appropriate services to achieve their goals.
File Management
- Ensure client intake process and secondary assessments are consistently and appropriately administered.
- Responsible for reporting client data and outcomes on a consistent and regular basis.
- Responsible for integrity of client files and data entry in HMIS.
Additional Duties:
- Participate in regular staff meetings.
- Maintain skills through on-going training.
Education and Qualifications:
Bachelor's degree in social work or related field, 2-5 years experience.
Required Skills:
- Excellent written and verbal communication skills.
- Bilingual (English/Spanish) candidates strongly preferred.
- Ability to work well independently and on a team.
- Ability to work with people of diverse backgrounds.
- Availability, dependability and willingness to be flexible.
- Demonstrated ability to solve problems and think strategically.
- Excellent computer skills including proficiency in Microsoft Word, Excel, and data entry.
- Detail oriented.
Education and Qualifications:
Bachelor's degree in social work or related field, 2-5 years experience.
Physical Requirements:
This position requires sitting, standing, driving and lifting as much as 50 lbs.
Hours and Location:
Full time, generally business hours but case managers may need to meet with clients in evenings or on weekends. Travel required to clients' locations. Case managers work out of one of our southwest suburban Cook County offices (La Grange, Summit, Oak Lawn). Some remote work may be done.
