Our ACO is looking for someone with a positive attitude, efficient in active communication and high energy individual who presents a high level of strong interpersonal skills and an efficient communicator.
•Qualifications: Minimum of an associate degree. Presents with basic understanding of medical terminology and good command over basic computer skills. Ability to learn to navigate various EMR/EHR systems.
•Experience: Office staff experience in a medical office or health care related field. Experience in engaging with patients and medical staff office.
Responsibilities
•The main focus of this position is contacting eligible patients to inform and educate them on the Medicare's Annual Wellness Visit benefit and to coordinate with provider office staff to schedule these patient’s AWV, Transitional Care Management visits and follow up visits with the patient's Provider.
- Helping patients on navigating Medicare website.
•By performing outreach activities, the Member Outreach Consultant will act as a representative of our Client Practices, providing excellent customer service to our patients and improving quality in patient care.
•Comfortable collecting and tracking quality measure metrics and managing team to certain performance expectations.
Benefits
•Estimated Salary: $20 to $25 per hour based on qualifications. Performance-based bonuses are also available.
