SUMMARY:
The Forensic Assertive Community Treatment (FACT) Program provides support to individuals with mental health and substance use disorders who are involved in the criminal justice system. The program focuses on diversion and re-entry, providing comprehensive, community-based treatment and services aimed at reducing recidivism and improving overall quality of life.
In this role, you will work on a multidisciplinary team and be responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, managing and administering medication in conjunction with the team psychiatrists, and providing a range of treatment, rehabilitation, and support services. This hands-on approach is crucial for building trust, understanding individual needs, and effectively aiding participants in their journey toward stability and integration into the community.
This position is based out of our San Jose Office and will provide direct client services in the community.

Required:
- Licensed by the California Board of Registered Nursing.
- 3 years of experience in clinical settings and broad understanding of psychiatric medication and mental health treatments and services.
- Injection administration.
Preferred:
- 2 years of experience as an RN.
- Experience with Electronic Medical Records (EMR) systems.
- Working knowledge of Santa Clara County and surrounding area community resources.
Knowledge:
- Knowledge of professional nursing principles and techniques used in the assessment, care and treatment of psychiatric patients.
- Knowledge of the pharmacological process in the appropriate administration or delivery of medications used in the care and treatment of psychiatric patients.
- Comprehensive knowledge of medical terminology used in the assessment, care and treatment of psychiatric patients.
- Comprehensive knowledge of laws and regulations covering nursing practices as it relates to care and treatment of psychiatric patients.
What You'll Be Doing in the role of Registered Nurse:
- Participate in daily staff organizational meetings and treatment planning review meetings.
- Collaborate with the clinical team and management on triaging and supporting the highest-need clients.
- Partner with the rehabilitation services team, primary care doctors, and community medical partnerships to provide medical care.
- Provide medication education to clients and team members.
- In collaboration with the team psychiatrists, assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the FACT team.
- Provide medical administration to clients and monitor and facilitate physical health care.
- Provide clinical care and crisis intervention in the community.
- Coordinates and oversees medical treatment, initiating appropriate health care referrals.
- Collaborates with physicians, psychologists, social workers, the clinical treatment team, and/or other professionals to discuss treatment plans and progress for clients to ensure healthcare requirements are met.
- Participate in and conduct parts of the client-centered comprehensive assessment of psychiatric history, physical and dental health, use of drugs and alcohol, and other history assessment areas.
- Consult with community agencies and families to coordinate the treatment process.
- Participate in treatment, rehabilitation, and support services.
- Provide symptom education to enable clients to identify their mental illness symptoms.
- Coordinates, oversees, evaluates and implements activities and functions associated with ordering, monitoring, and handling medications (Medical Doctor (MD) orders / Vital Signs (VS) / Medication monitoring).
- Maintains inventory control of medications, distribution and standardization of dosages.
- Orders laboratory or diagnostic tests as prescribed by the physician. Reviews findings and alerts physicians.
- Coordinates pharmacy orders to ensure medication is being sent to correct pharmacy.
- Assist with medical appointments and may require attendance to appointments in conjunction with FACT team.
- Provide direct client services, including crisis services during scheduled shifts in the community, including but not limited to parks, community spaces, transitional housing units, rehabilitation facilities, or board and care facilities.
- Assists and supports staff in interpreting and applying physician orders. Follows physician’s orders, monitors patient status, and reports to physician.
- Supports staff with addressing health care concerns, evaluation, and interface with physicians and external providers.
- Maintains and manages medical and psychiatric records in an organized and timely manner so as to meet the standards of care, accreditation, licensing requirements, and resident care needs.
- Completes all electronic medical records system documentation, including incident reports and progress notes within the standardized time frame.
- Compliant with the standardized Medication Practice Guidelines and Clinical Record Documentation Manual for Outpatient Services provided by Santa Clara County Mental Health Department
- Assists with quarterly Pharmacy Audits for the Agency.
- Supports Psychiatrist by preparing client requested forms including County and State applications requiring mental status evaluation. Prepares letters as requested by the client for school, housing, or other requirements.
- Performs other related responsibilities, as assigned, to support specific department/business needs.
- Responds to phone calls and emails within 24 hours on business days. Resolves outstanding issues collaboratively using strength-based solutions focused approach.
- Demonstrates an understanding of HIPAA, confidentiality laws and policies, and assures the highest degree of confidentiality in the implementation of all responsibilities and duties.
- Participates in internal and external meetings (County, community, and agency and team meetings), as requested. Attends regular meetings with physician staff and treatment teams.
- Successfully completes all training requirements for this position to ensure agency and contract compliance.
- Ensure medication storage and destruction complies with county and stated licensing standards.

- Must be able to meet and receive a criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices.
