Job Overview:
Under the guidance of the Medical Director, the Primary Care Provider will work closely with other healthcare professionals and the IDT to perform assessments, plan care, and provide medical education for participants in the program. This role requires a high level of expertise and a compassionate approach to patient care.
Key Responsibilities:
- Conduct in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, ensuring continuous medical care for participants.
- Collaborate with physicians and the IDT to develop individualized care plans.
- Assist in discharge planning from acute or long-term care settings, coordinating with various healthcare professionals.
- Maintain care for participants in nursing facilities, making regular visits as required.
- Work closely with Our Award-Winning Client physicians, community-based PACE physicians, and the IDT to ensure effective care coordination.
- Review outcomes and utilization data, including hospitalizations and specialist use, to help establish best practices and policy improvements.
- Participate in rotating night and weekend on-call duties.
Job Requirements:
- Graduation from an Accredited School of Nursing
- Certificate as a Nurse Practitioner with Prescriptive Authority
- Nurse Practitioner State License
- A minimum of two years of experience working with the senior population
- Current knowledge of chronic care and geriatric issues, as well as best practices in the field
Technical Skills and Certifications:
- Certified as a Nurse Practitioner with Prescriptive Authority
- Licensed in California
- Experience in Geriatrics (preferred)
- Two years of experience as a Nurse Practitioner
