**Job Overview:**
Autumn Hospice is growing and recently awarded the Norwell Chamber's Rising Star award for the amazing work they are doing in their Norwell and Worcester markets. We are seeking a dedicated Hospice Registered Nurse to join our team. The ideal candidate will be responsible for coordinating patient care, providing quality end of life nursing , and ensuring strict compliance with hospice COPs.
**Duties:**
-In conjunction with the Interdisciplinary Team (IDT), manage and coordinate the patient and family/caregiver Plan of Care from referral to discharge.
- Facilitate communication about changes in the patient’s status among IDT members and the patient’s attending physician.
- Apply the policies and procedures of Hospice and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing hospice care.
Conduct comprehensive assessments and develop individualized care plans
Participate in the on-call nursing rotation schedule.
Attend deaths of patieints to assist and support family/caregivrs
- Plan, coordinate, and deliver direct nursing care to patients and families according to the patient’s need.
- Serve as a patient advocate while promoting independence and self-care
- Provide education to patients and families on disease processes, comfort care and self-care techniques
**Requirements:**
- Valid RN license
- Bachelor's degree in Nursing (BSN) preferred
- Previous experience in hospice, post acute care, or nursing home setting
- Ability to work independently with remote supervision available
- Strong clinical skills in patient assessment
- Excellent communication and interpersonal skills
- Ability to inhale under pressure and make critical decisions

Pay: $85,000.00 - $95,000.00 per year
