We are seeking a dedicated Registered Dietician to work prn. The registered dietician provides an assessment of the patient’s nutritional status, instruction, and counseling to staff, patients, and caregivers/families. Candidate must reside in the state of Illinois.
Essential Duties and Responsibilities:
- Conducts patient assessments to determine nutritional needs and develops appropriate dietary plans as requested.
- Educates patients and families on proper nutrition, including meal planning, portion control, and food choices. Reevaluates nutritional care by providing follow-up care, primarily by phone or at the patient’s residence or facility.
- Provides professional, courteous service to the patient consistent with knowledge, training, and experience.
- Collaborates with healthcare professionals to ensure coordinated care and effective treatment plans.
- Develops, implements, evaluates, and adjusts the nutritional plan based on an assessment of nutritional needs as part of the patient’s total plan of care reporting results as directed.
- Educates interdisciplinary group (IDG) members regarding the technical nutritional needs of hospice patients. Attends interdisciplinary group meetings as needed.
- Records dietary history, nutritional assessment, treatment plan and progress notes in the patient’s electronic medical records.
Education:
- Bachelor’s Degree from an accredited institution with major in the field of nutrition, or dietetics with courses in food preparation and service; methods and principles of teaching.
- Current Illinois license
- Certification from the American Dietary Association preferred.
- Minimum two years of experience in a related field of dietary work. A satisfactory combination of education, training, and experience will be considered.
- Valid Driver's License, auto insurance coverage, and have access to a reliable automobile.
