The evaluation of work performance is an ongoing process. The purpose of the job description and written performance appraisal is to document a summary of the individual’s performance in relationship to the job description, responsibilities, and standards and encourage identification of goals. This document serves the purpose of the job description and performance evaluation.
Job Summary
The Case Manager is responsible for the administration and direct services of the Rapid Rehousing and Street Outreach Programs of the YWCA of Clinton Empowerment Center.
Job Responsibilities
- Strong working knowledge of computers
- Maintain effective working relationships with community resources, both internal and external
- Responsible for accurately entering client data into HMIS system
- Coordinate transportation when necessary
- Maintain a working knowledge of community resources
- Develop relationships with area landlords in order to facilitate client placement
- Maintain housing directory which should at minimum include documentation of all contacts with housing providers including: corporations name, name of contact, address, phone, date of contact, and result of contact
- Participate in meetings and trainings as required
- Complete participant files within required deadlines
- Complete all HUD documentation and maintain files for review
- Maintain detailed, up to date, and concise case notes on all interactions with participants and on behalf of participants
- Responsible for HUD service coordination caseload of residents in the Rapid Rehousing Program and Street Outreach Programs.
- Assist clients in accessing mainstream resources and community resources
- Assist clients who are living in places not meant for habitation
- Identify, assess, plan, stabilize, and support clients in the Rapid Rehousing Program and Street Outreach Programs
- Assist participants in the housing search process, included helping participants identify suitable housing options, accompanying participants to view units and advocating for participants with landlords, and assisting clients with paperwork.
- Connect clients to services; advocate for clients with other service providers
- Represent the Empowerment Center at events or on committees
- Have a knowledge and understanding of Trauma Informed Care.
- Travel into Clinton County and Jackson County to meet with clients.
- Other responsibilities as assigned

Requirements
- BA in Social Work, Human Services or related fields and/or experience.
- Those with lived experience are encouraged to apply.
- Annual Bloodborne Pathogens
- Maintain Driver’s License
- Maintain Adequate Insurance
- Criminal /child abuse record check
Job Type: Full-time
Pay: $15.00 - $18.00 per hour
