Responsibilities:
Physical Therapist
Summary:
Provides Lymphdema physical therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of physical therapy and within AxiomHealth Management guidelines. Provides quality patient care including screening and evaluation of patients, development of initial and ongoing treatment plans of care, supervision of assistants, and case management responsibilities. Maintains professional conduct that represents Axiom Health’s commitment to quality and service in all interactions with patients, including positive levels of interaction and rapport with facilities and clients. Adheres to all state and federal regulations regarding physical therapy.
Essential Duties and Responsibilities:
- Perform clinical assessments and identify patient needs and appropriateness of care
- Provide consultation and clinical supervision to PTAs.
- Document, prepare and complete a plan of care, progress notes, phone orders and other clinical record documentation on an ongoing basis
- Revise the plan of care based on evaluation and ongoing assessment data
- Perform and oversee other team members, when applicable, in the performance of patient care that is consistent with patients’ needs and desires
- Accepts clinical assignments that are consistent with education and competence to care for patients
- Plan and provide appropriate treatment for Lymphedema
- Apply concepts of infection control and standard precautions in coordinating/performing patient care activities
- Meet mandatory continuing education requirements of the agency/licensing board
- Promote agency philosophy and administrative policies
- Demonstrate commitment, professional growth, and competency
- Enhance clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self-study, and other continuing education activities.
- Maintain a valid state license. Maintain awareness of issues related to the profession of physical therapy and the health care environment.
- Be compliant with infection control procedures and environmental safety protocol within a facility.
- Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports
- Bilingual a plus
Qualifications:
Education/Experience:
- A graduate of an APTA accredited school of physical therapy
Computer Skills:
- Computer literate and proficient in clinical documentation
- Basic Internet navigation skills
Certificates and Licenses:
- Valid physical therapist assistant state license
- Current Driver's license
