Qualifications:
LVN License Required***
- LVN’s must have and maintain a current Texas nursing license. It is the responsibility of each nurse to keep up his or her own required continuing education hours
Position Summary:
Manages the day-to-day health and wellness operations of the community to ensure resident’s needs are met.
Supervisory Responsibilities:
Directly supervises resident care team members for the Community.

- Meets SilverPoint’s standards established for the resident care department.
- Assists in the community staffing of resident care team members, medication technicians, and/or nursing staff.
- Provides training and education to team members on an on-going basis to include classroom in-services and if required, situation specific training.
- Assists in the maintenance of resident charts and review of documentation performed by resident care team members.
- As allowed by the Nurse Practice Act, assists in assessing the health, functional, and psychosocial status of residents. Initiates individualized care plans and proactively manages care and services for each resident. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment.
- Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, and identifying risks for move-outs.
- Oversees and manages the consistency of medication training, pharmacy management and medication supervision and/or administration within the community.
- Conducts periodic medication technician medication skills, inventory checks and periodic medication audits.
- Monitors and participates in care plan meetings and family conferences.
- Communicates in residents’ health and well-being, personal needs, risk management issues, resident issues, family or outside healthcare provider concerns.
- Assumes on-call responsibilities as needed and remains available to team members at all times by phone to answer questions regarding residents.
- Assists in planning and/or preparing for community special events, including outreach to the greater community.
- Attends and actively participates in team meetings and required or optional continuing education programs.
- Assists in the evacuation of residents as necessary.
- Maintains the confidentiality of all resident information and honors the residents’ personal property rights.
- Performs any other duties that may be assigned by the Executive Director.
- Performs other duties as assigned and which relate to the success of SilverPoint or the Community.
Line of Authority:
The Health & Wellness Director reports to the Executive Director.
Job Type: Full-time
Pay: $70,000.00 - $75,000.00 per year
