Hybrid remote work in Klamath Falls, OR 97601
This position focuses on decreasing health inequities and increasing member access to SDoH services and supports through care coordination activities. This position also identifies members with SDoH needs through several methods, including member and community-level data sources, internal referrals from Case Managers and Utilization Reviewers, the completion of Health Risk Assessments (HRAs), and daily monitoring of hospital and jail reports. This position utilizes member HRA data to make appropriate internal referrals to RN and/or BH Case Management for non-SDoH needs, and coordinates care with external services and supports for members with SDoH needs. This position also supports the Health Equity team in the Flex Funds and Health Related Social Needs (HRSN) authorization process from start to finish and offers member-level SDoH information and insights to support the successful completion of Health Equity-related initiatives and deliverables by performing the following duties.
JOB DUTIES
- Supports the Health Equity department in the Flex Funds and HRSN process from start to finish.
- Coordinates care for members identified with SDoH needs.
- Participates in Interdisciplinary Treatment (IDT) and care collaboration meetings with network partners to identify members with SDoH needs and to receive SDoH member referrals.
- Leverages partnerships with contracted partners who provide SDoH resources and supports to assist with care coordination of members with SDoH needs.
- Maintains a current and extensive knowledge base of SDoH and Health Equity-related disparities our members may face in multiple settings and cultural contexts, acting as CHA’s member-level SDoH Subject Matter Expert (SME).
- Maintains an extensive knowledge base and understanding of Oregon Administrative Rules and Oregon Health Authority (OHA) initiatives as they relate to SDoH, supporting compliance in these areas.
- Assists members and coordinates with other departments for outreach efforts with education and completion of HRA and SDoH assessments.
- Identifies non-SDoH needs through HRA and SDoH assessments and makes appropriate internal referrals.
- Makes appropriate external referrals for SDoH needs when identified.
- Works within the Quality & Health Equity department to reduce health inequities through participation in identifying member-level data points to be collected, tracked, and reported through SDoH care coordination process.
- Supports the Health Equity Manager by ensuring assigned SDoH-related requirements are met for OHA incentive metrics, Transformation and Quality Strategy projects, and Performance Improvement Plans.
- Participates in member outreach activities and events that address SDoH needs.
- Supports the Health Equity manager in creating and maintaining SDoH policies and procedures, and other relevant written materials.
- Maintains confidentiality and complies with HIPAA rules and regulations.
- Maintains punctual, regular, and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Displays excellent communication skills including presentation, persuasion, and negotiation skills required in working with members and coworkers, including the ability to communicate effectively and remain calm and courteous under pressure.
- Respectfully takes direction from manager.
Bilingual (Spanish/English) is preferred.
EDUCATION and/or EXPERIENCE
Associate’s degree from two-year college or university; and 2to 4years related experience and/or training; or equivalent combination of education and experience.
COMPUTER SKILLS
Job requires specialized computer skills. Must be adept at using various applications including database, spreadsheet, report writing, project management, graphics, word processing, presentation creation/editing, communicate by e-mail and use scheduling software.
CERTIFICATES, LICENSES, REGISTRATIONS
This position requires OHA certification as a Traditional Health Worker or the ability to obtain within six months of employment.
