We're thrilled to announce a dynamic opportunity for you to become an integral part of our Houston, Texas team. As an onsite contributor, you'll play a key role in pioneering a pathway toward a more streamlined and effective healthcare system.
JOB SUMMARY:
The Care Coach, a nationally certified medical assistant, engages directly with patients through one-on-one encounters and group sessions. Their primary role is to support patients in managing their chronic conditions and achieving their established care plan goals.
DUTIES AND RESPONSIBILITIES:

Conduct face-to-face visits with patients to promote and reinforce established care plan goals, including education on lifestyle modifications.

Collaborate with the Care Educator Nurse to discuss cases and ensure the closure of patient care gaps.

Identify and attend to same-day patients who can benefit from participation in the care education program.

Facilitate patient group workshops as coordinated by the Care Program Nurse Manager.

Accurately document patient information within the Electronic Medical Record (EMR) system.

Participate in health fairs and other events aimed at promoting health within the community.

Fulfill any additional duties assigned by the Care Program Nurse Manager.
Requirements:
- National certification as a medical assistant.

CPR/BLS required.

Two years of medical assistant experience preferred.

Bilingual (fluency in speaking Spanish) is preferred.

Strong interpersonal skills and ability to engage effectively with patients.

Knowledge of chronic condition management and lifestyle modification techniques.

Proficiency in documenting patient information in an EMR system. Epic experience highly preferred.

Ability to collaborate effectively within a multidisciplinary team.

Flexibility to adapt to changing priorities and patient needs.
