Job Description:
Description: The Patient Care Coordinator-Pharmacy Operations will support JPS pharmacy patient throughput and Pharmacy Team with coordination of safe and efficient patient prescription delivery. The Patient Care Coordinator-Pharmacy Operations will work with multi-disciplinary teams including pharmacy, nursing, case management and other roles as needed.
Typical Duties:
- Monitor patient prescription orders and ensure proper prioritization within pharmacy operations

Assures timely consistent communication with care team and pharmacy

Verifies patient demographic information and educates patients of delivery options

Monitors prescription care plan for patients and works with multidisciplinary team to eliminate barriers
Tracks prescription orders via pharmacy tracking program to ensure correct delivery information
- Assists in problem solving in matters relating to the pharmacy patient prescriptions

Monitors patient’s prescription delivery to ensure medications are delivered or picked up timely

Cooperates in maintaining a clean, orderly, safe, and professional environment

Provides technical support of drug distribution services for all outpatients

Ensures medication management processes meet all legal and regulatory requirements. Assists with monthly reporting

Participates in performance improvement (PI) activities which improves patient care, specifically best in class pharmacy processes

Performs cashier functions and daily cashier reports, if needed

Collects pharmacy copays according to determined financial classification and 3rd party eligibility

Performs other related duties as required
Qualifications:
Required Education and Experience:
- High School Diploma, GED or equivalent.
- 6 months of healthcare experience
Preferred Education and Experience:
- 3 plus years of pharmacy experience
Required Licensure/Certification/Specialized Training:
- Current pharmacy technician registration with the Texas State Board of Pharmacy.
- National certification recognized by the Texas State Board of Pharmacy. National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB).
