LINCENSED PRACTICAL NURSE (LPN)
Responsibilities:
- On-Going Patient/Family Education
- Initial/On-Going Patient Assessment
- Health Management
- Medication Management
- Chronic and Acute Disease Management
- Interdisciplinary Collaboration with Care Teams
- Management of Home-Health Plan of Care
- Management of Skilled Goals and Interventions
- Direct Patient Care / Nursing Skills: Medication management, education and reconciliation; Wound care (basic and advanced, NPTW, etc.); Drain care; Cath care; -Ostomy care; VAD management and VAD site care; Venipuncture and blood sample collection via VAD; Etc.
Requirements:
- Graduate of and Accredited Practical Nursing (LPN) Program
- Active LPN License in Good Standing (RI/MA)
- Minimum One (1) Year Nursing Practice
- Infusion Nursing Experience (Preferred)
- Home-Health Experience (Preferred)
- Independent Assessment Skills
