Job Overview:
We are seeking a dedicated Discharge Scheduler/Case Manager to join our team. The Case Manager will be responsible for coordinating patient care and providing support to individuals in need. This role requires a compassionate individual with strong organizational skills and a background in healthcare services.
Responsibilities:
- Collaborate with healthcare professionals to develop and implement patient care plans
- Conduct assessments to identify patient needs and develop appropriate interventions
- Maintain accurate and up-to-date patient records
- Provide emotional support and guidance to patients and their families
- Ensure compliance with HIPAA regulations and medical coding standards
- Coordinate referrals to specialists and other healthcare providers
- Monitor patient progress and adjust care plans as needed
- Leads and implements the discharge planning process for patients within a health facility
- Provides education and support to the patient and hospital staff in the development and implementation of discharge plans
- Plays a case management role in helping released patients develop continuing care plans
- Organizes an effective post-treatment strategy for patients to heal fully at home or another care facility
Qualifications:
- Proficiency in medical terminology and medical records management
- Experience in pediatrics, patient services, social work, or behavioral health is preferred
- Knowledge of case management principles and practices
- Familiarity with HIPAA regulations and confidentiality protocols
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced healthcare environment
