Responsibilities:
· Participates in the Interdisciplinary Team’s initial assessments, care planning and on going re-evaluations of participant’s case.
· Maintains current written case management records, including periodic reassessments of program participants.
· Provides individual and family counseling, develops and leads group counseling and activities.
· Refers participants to appropriate community agencies or facilities, acts as a liaison with such organizations and as advocate for participants.
· Encourages effective relationships amongst staff geared to the building and maintenance of a cohesive team.
· Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
· Conducts periodic assessments.
· Communicates participant changes to team members.
· Participates in the development of the IDT Plan of Care.
· Participates in the coordination of a 24 hour care delivery.
· Completes all documentation in the medical record as required.
· Only act within the scope of his or her authority to practice.
· Meet standardized set of competencies for the specific position description established by PACE @ Home and approved by CMS before working independently.
- Demonstrates familiarity with the personnel policies of PCHCV and understands and adheres to rules, regulations and standards of regulatory bodies and applicable accrediting agencies of PACE @ Home.
- Complies in a timely manner to all mandatory inservice requirements of Pace @ Home.
· Accepts other assignments as appropriate.
Qualifications:
Master’s Degree from an accredited Schoolof Social Work. Must have the following licensure, registry or certification: MSW, LCSW. Must maintain CPR and First Aid Certification.
Two years experience in a health related area and one years experience working with the frail and elderly population.
· Ability to work effectively in a team environment.
· Excellent verbal and written communication skills.
· Individual therapy skills geared to geriatric patients.
· Work performed includes visits to private homes of PACE enrollees. Position requires frequent travel in and around the Catawba Valley area.
· Able to work with adult geriatric patients.
· Ability to manage time efficiently and prioritize duties.
· Ability to work with people from different cultures, ethnic back grounds and religious affiliations.
· Ability to effectively handle difficult and unusual interpersonal situations.
· Consistently demonstrates responsibility, accountability and dependability.
· Proficient computer skills with MS Office products.
· Excellent customer service and communication skills.
· Maintains current, unrestricted driver’s license.
· Maintains minimum automobile liability insurance for duration of employment.
