Job Summary:
- Responsible for the coordination of care for five patients in an inpatient hospice setting.
- Provides skilled nursing to hospice patients utilizing nursing assessment, planning, implementation, and evaluation.
- Develops and/or updates a plan of care for the hospice patient and their caregivers in collaboration with the physician and interdisciplinary team.
- Completes admission of patients as needed. Utilizes principles of palliative and hospice philosophy to provide holistic care.
- Maintains appropriate communication with administrator and physician on call. Provides individualized patient/family education.
- Maintains accurate and timely documentation in the electronic medical record.
- Provides direct supervision of Certified Nurse Assistants (CNA's).
- Handles crisis situations in a professional manner.
Qualifications:
- Graduate of an accredited school of nursing. BSN preferred.
- Current HI RN license, in good standing with the State licensing board.
- CPR certification.
- Two (2) years of experience as an RN to include hospice, home health, nursing facility or acute care.
- Experience with pain and symptom management to include infusion pumps.
- Demonstrate excellent assessment, critical thinking, time management and communication skills along with cultural sensitivity and a deep understanding of the dying process.
- Previous experience with electronic medical record systems.
- Ability to lift, position and transfer patients. Considerable reaching, stooping bending kneeling and crouching required. Prolonged or considerable walking and standing required. Visual acuity and hearing to perform required nursing duties.

Pay: $44.18 - $46.69 per hour
