The home health aide provides personal care and related services in the home. She/he functions under the direction, instruction and supervision of the RN and the CQI Supervisor.
Reports to: RN Case Manager and Office Manager
Education/Qualifications
- Current Ohio Certification of an Home Health Aide
- Minimum of one (1) year hospital experience and one (1) year home care experience.
- Two (2) satisfactory references from past employers.
- Demonstration of satisfactory knowledge and competency for the delivery of services, treatments, and teaching under the direction of a Registered Nurse.
General Description
Delivery of patient care as appropriate to provide services in accordance with Agency policies under the direction of the Registered Nurse.
DUTIES/RESPONSIBILITIES
1. Tasks performed by an aide must be assigned by and performed under the supervision of an RN or PT who will be responsible for the client care provided by the aide.
2. The duties of the aide may include:
a) Help the client to maintain good personal hygiene
b) Assisting in maintaining a healthful, safe environment
c) Assisting with ambulation
d) Assisting with certain treatments as ordered by the physician and approved and supervised by the nurse.
e) Assist the therapy personnel as needed with rehabilitative processes.
f) Encourage the client to become as independent as possible according to the nursing care plan.
g) Attempts to promote client’s mental alertness through involvement in activities of interest.
h) Gives emotional and psychological support to the client and other members of the household.
i) Establishes relationship with client and family which transmits trust and confidentiality
j) Prepares a report of her visit on the day it is performed and incorporates same in clinical record weekly
k) Reports any change in the client’s mental or physical condition or in his/her home situation to the immediate supervisor.
l) Carries out assignment as instructed by the nurse and reports to the nurse when unable to do so.
m) Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the nurse.
n) Attends in-service as required by state and federal regulations.
o) Confirms on a weekly basis, the scheduling of visits with the Supervisor to coordinate necessary visits with other personnel.
Job Types: Full-time, Part-time
Pay: $11.00 - $14.00 per hour
Expected hours: No more than 40 per week
