Job Opportunity: RN Case Manager

Position Overview:
As an RN Case Manager, you will play a crucial role in providing comprehensive case management services to our members. You will work closely with interdisciplinary teams to ensure that our members receive the highest quality of care and support.
Key Responsibilities:
Conduct thorough clinical and psychosocial assessments of members
Develop and implement individualized care plans based on member needs
Coordinate care with healthcare providers and community agencies
Facilitate transitions of care for members moving between settings
Provide support and guidance to members and their families
Collaborate with team members on quality improvement projects
Attend and actively participate in meetings and training sessions
Requirements:
Active California RN License without restriction
Minimum three years of experience in case management or related field
Knowledge of managed care principles and practices
Strong clinical knowledge and critical thinking skills
Excellent communication skills
Ability to work effectively within an interdisciplinary team
Proficiency in computer software applications
Valid California driver’s license (or alternative transportation)
Ability to maintain confidentiality and comply with policies and procedures
Working Conditions:
This position primarily operates in an office environment, with occasional visits to member homes or facilities. Physical requirements include mobility, lifting, visual acuity, dexterity, hearing, and reasoning skills.
