We are seeking a dedicated and compassionate Behavioral Assistant to join our team, specializing in providing care and coordinating community activities for individuals requiring community support services. The ideal candidate will play a crucial role in enhancing the well-being and social engagement of those we serve.
Responsibilities:
- Develop and implement person-centered care plans based on individual needs, preferences, and goals.
- Provide direct care and support to individuals in various community settings.
- Plan, organize, and facilitate community-based activities and outings to promote socialization, skill development, and community integration.
- Establish partnerships with local organizations and resources to expand community engagement opportunities.
- Conduct assessments to identify strengths, challenges, and goals of individuals receiving services.
- Collaborate with clients, families, and interdisciplinary teams to set and review individualized goals.
- Implement behavioral strategies and interventions in collaboration with behavior analysts and other professionals.
- Foster a positive and supportive environment to enhance the social and emotional well-being of individuals.
- Maintain accurate and up-to-date documentation of client progress, activities, and any significant incidents.
- Generate regular reports for supervisors and interdisciplinary teams.
- Establish and maintain open communication with clients, families, and other service providers.
- Attend meetings and participate in case conferences to ensure coordinated and comprehensive care.
- Crisis Intervention:
- Respond to crisis situations using established protocols and procedures.
- Collaborate with the interdisciplinary team to develop and implement crisis prevention plans.
Qualifications:
- Bachelor's degree in a related field (Psychology, Social Work, Human Services, etc.).
- Previous experience in providing care or support services, especially in community settings.
- Knowledge of person-centered planning and the principles of Applied Behavior Analysis (ABA) is a plus.
