The Program of All-Inclusive Care for the Elderly, or PACE, was established by Medicare to help independent seniors with complex medical needs. The program helps participants avoid nursing homes and instead, receive the care and services necessary to help them stay safe, comfortable, and healthy living in their own home.

Requirements
1. Occupational Therapist license issued by the State of California Occupational Therapy Board required.
2. Prior experience working with the frail elderly (Geriatric) and disabled populations preferred.
3. Current CPR/BLS certification required.
4. Bilingual English/Spanish/Cantonese/Mandarin preferred.
5. Post-professional education and training in Physical Agent Modality preferred.
Responsibilities
1. Assess, plan, prepare and carry out individually designed programs of occupational therapy treatment to maintain, improve or restore physical functioning, enhance emotional and social well-being, alleviate pain and prevent physical dysfunction in patients.
2. Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit and to meets the patient's goals and needs.
3. Help patient develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving manual arts and crafts, practice in functional, prevocational, vocational and homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities.
4. Supervise all treatment/graded activities carried out by assistants/aides.
5. Design home exercise programs, recommend and/or provide assistive device, instruct patients, families, and caregivers in home exercise programs that, if followed, will assure continuation of therapeutic plan following discharge from skilled therapy.
6. Direct or supervise group rehabilitation activities.
7. Provide information to the participant about the proposed intervention, its material risks and expected benefits and any reasonable alternatives.
8. Document in accordance with PACE departmental guidelines and within the established time frames.
9. Complete assessments and collaborate with physicians, nurses, social workers, and other health care workers to establish an integrated individualized plan of care.
10. Develop rehabilitation aides by providing information, guidance, training and conducting in-service training programs.
11. Maintain patient confidence by keeping information confidential.
12. Maintain safe and clean working environment by complying with company policies and procedures, rules, and regulations.
13. Protect patients and employees by adhering to infection-control policies and protocols.
14. Comply with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
