Position Summary
The Behavioral Health Clinician, as part of the primary care treatment team, identifies, triages and assists in management of patients with medical and behavioral health problems within the primary care setting.
**Position will be split 20 hours per week at SE 82nd Clinic and 20 hours at ourTigard Clinic**
Position Responsibilities
Common responsibilities of Behavioral Health Clinicians include (but are not limited to):
- Screen and assess patients for psycho-social issues, as well as common mental health and substance abuse.
- Provide patient education about common mental health and substance abuse disorders and available treatment options.
- Support and closely coordinate mental health care with the patient’s primary care provider and, when appropriate, other treating providers.
- Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment.
- Provide brief interventions using evidence-based techniques.
- Facilitate patient engagement and follow-up care.
- Track patient follow-up and clinical outcomes using the electronic health record (EHR).
- Document in-person and telephone encounters in the EHR and use the system to identify and re-engage patients.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP.
- Provide basic support and/or facilitate referrals to patient resources and/or complex case management for clinically indicated services outside the primary care setting.
- Facilitate in-clinic or outside referrals to evidence-based behavioral health services as clinically indicated.
- Follow all protocols defined by Health Share of Oregon for Behavioral Health Integration in the primary care setting.
- Other duties as assigned.
Qualifying Behavioral Health Clinicians
- Licensed psychologist, licensed clinical social worker, or licensed professional counselor in Oregon
- Ability to be covered by medical malpractice insurance and credentialing
Key Competencies and Skills
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Ability to make quick and accurate clinical assessments of mental and behavioral health conditions.
- High comfort in working with primary care medical providers using a consultative style.
- Ability to work through brief patient contacts, including via telephone and teleconferencing.
- Good knowledge of psycho-pharmacology.
- Ability to design and implement clinical pathways for protocols for treatment of selected mental and behavioral conditions.
- Ability to train and teach effective behavioral health interventions and the primary behavioral health model to medical practitioners and other behavioral health providers.
- Ability to work effectively as a team, interfacing with patients, primary care providers, consultingpsychiatrist and mental health specialists, as well as with administrative and support staff.
