Under the direction of the Nursing Supervisor, (.75 or 30 hours weekly) of work responsibilities involve providing comprehensive, person-centered, team based, community treatment in the Comprehensive Community Services program to persons diagnosed with mental illness or substance abuse or dependence. Work includes the provision of: service facilitation, symptom management, medication management/med set up, physical health monitoring, psychosocial rehab, and skill development according to individualized Recovery plans. Responsibilities (.25 or 10 hours weekly) also include providing supportive nursing services and medication set up for the Rock County Outpatient Clinics and the Crisis Stabilization Unit . In conjunction with direct service provision in both the CCS program, Outpatient Clinics and the Crisis Stabilization Unit, responsibilities include the development and maintenance of client case records, medication records, responsibility for tracking, counting, and setting up client medications, and report/documentation completion in accordance with DHS 36, 35, 34, Social Security and Medicaid/Medicare. Work involves collaboration with a treatment team, other HSD employees, families and collateral agencies. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers."
45%- Nursing activities for the CCS program as detailed:
1. Medication Management - setting up of medications, taking and filling orders, medication reconciliation after hospitalization (medical or psychiatric), medication packet reconciliation each week/med set up, monitoring of medication efficacy and side effects
2. Physician supports - interim medication checks, side effect monitoring, efficacy monitoring, collateral contacts with primary medical doctors (PMD) including coordination of medical treatment/taking medication orders from PMD/medical providers, review of laboratory reports (including Clozapine), coordination of lab orders, injections, lab draws, coordination with program psychiatrist when psychiatric issues present while MD out of the office
3. Nursing assessment - monitoring vitals, monitoring weights, patient education, management of mild medical issues, provide direction on when to seek ER/urgent care/PMD appointment, DISCUS completion/tracking
4. Facilitate consumer groups and provide education related to health and medication.
20%- 1. Supportive nursing activities, not to exceed 10 hours weekly provided to the Rock County Outpatient Clinics and Crisis Stabilization Unit.
2. Includes: medication management, physician support, and nursing assessment as described in the category above.
3. Coordination with outpatient and crisis stabalization supervisor, program manager, and staff to assure that medications are prepared according to client plans.
10%- Provide comprehensive psychosocial rehab services that are person-centered, team oriented, and community based in accordance with DHS 36.
1. Manage a case load and provide or arrange for provision of all needed services for each person on that case load. Develop and implement clinically relevant recovery plans ensuring that services are culturally competent, recovery oriented and involve consumer participation.
2. Provide interventions including; assessment, service planning, service facilitation, medication management, physical health monitoring, skill development, employment related skills training, psychoeducation, and recovery support services. Provide services on an individual and group level including possible supportive services and education to families. Provide a variety of psychosocial rehab interventions for individuals across the lifespan.
3. Advocate for and arrange transition to the least restrictive living situation for all CCS consumers. Attend and testify at court proceedings as needed.
4. Act as a team member on the CCS treatment team contributing to the functioning of the treatment team, supporting and covering for other team members as necessary.
5. Regularly assess consumer mental status, areas of functioning, achievement towards stated goals, and symptom patterns and communicate this assessment to the team.
10%- Work in collaboration with a treatment team, other HSD employees, families and collateral agencies.
1. Actively participate in treatment team meetings, individual and group supervision meetings, and treatment plan meetings for consumers on your case load and team.
2. Attend in-services, trainings, division/department meetings, committee meetings and trainings as requested or approved by your supervisor.
3. Maintain prompt communication with family members, team members, Rock County Crisis Intervention, and collateral agencies requesting information.
7%- Documentation of treatment progress notes for CCS in accordance with DHS 36, Medicare/Medicaid. Includes; orders, consultation with doctors/treatment providers/hospitals, documentation required for medication maintenance, Patient Assistance Program coordination for clients, insurance, prior authorizations, prescription refills, assistance with informed consents for medications.
5%- Documentation of treatment progress notes for Outpatient and Crisis Stabilization supportive services in accordance with DHS 35, 34 and Medicaid/Medicare. Includes; orders, consultation with doctors/treatment providers/hospitals, documentation required for medication maintenance, Patient Assistance Program coordination for clients, insurance, prior authorizations, prescription refills, assistance with informed consents for medications.
3%- Other duties as assigned.
