Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Care Partner/Case Manager in Worcester, MA.
CHL’s Health Home Management department provides care management to CHL clients who are also enrolled in Commonwealth Care Alliance’s OneCare program. We help coordinate these individuals’ primary and behavioral health care to ensure they receive high-quality, integrated care.
Case Managers work with our clients and their health care providers as a team to create an individualized personal care plan for each client, and then follow-up with the clients and providers regularly to ensure the care plan is meeting the client’s health needs.
CHL’s Health Home Management department offers staff an opportunity to learn about a variety of topics in health care, including insurance authorizations, long-term home services, primary care, behavioral health, addiction treatment, and hospital care.
Responsibilities
· Perform comprehensive intake and assessment of clients to determine the needs of the client including medical, psychiatric and wellness needs.
· Develop appropriate care plans in conjunction with clients for the delivery of essential services.
· Work collaboratively with multi-disciplinary team across providers and locations to coordinate the delivery of appropriate clinical and other support services.
· Make appropriate referrals to existing community agencies for additional services as identified in the assessment and care plan.
· Monitor the effectiveness of the care plan and coordinate with team to make changes to facilitate positive health outcomes and the prudent use of resources.
Qualifications
· Grade C02 Case Manager I: Requires HSD/GED/HiSet, Associates preferred.
· Grade C04: Case Manager II: Requires a Bachelor’s degree; preferably in the field of psychology or human services.
· Knowledge of homelessness and related issues, familiarity with community resources and entitlements (i.e., substance abuse and mental health).
· Bilingual/Bi-cultural English/Spanish strongly urged to apply.
· Current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work related purposes.
